Statement of accuracy: Include the following statement: "The information in this notification is accurate, and under penalty of perjury, I am the owner, or an agent authorized to act on behalf of the owner, of an exclusive right that is allegedly infringed."

Your signature: The notice must be signed by the person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed. Signatures may be provided electronically by typing your name.

Please be aware that under 17 U.S.C. § 512(f), you may be liable for any damages, including costs and attorneys' fees incurred by us or our users, if you knowingly materially misrepresent that material or activity is infringing. We will review and address all notices that comply with the requirements above. If we remove or disable access in response to such a notice, we may notify the owner or administrator of the affected content so that they can make a counter notification. We may also document notices of alleged infringement on which we act.
